Metastatic Brain Tumors

Primary Brain TumorsSecondary brain tumors, known as metastatic brain tumor, are caused by cancer that has spread from another part of the body, known as metastasis. They are different to primary brain tumors. They are much more common and always considered to be malignant.

Cancer that spreads to the brain is the same disease and has the same name as the original (primary) cancer. For example, if colon cancer spreads to the brain, the disease is called metastatic colon cancer because the cells in the secondary tumor resemble abnormal colon cells, not abnormal brain cells.

The most common types of metastatic tumors in order of frequency are: 1) lung cancer, 2) breast cancer, 3) renal cell cancer, 4) melanoma, and 5) colon cancer.

The behavior of these tumors varies hugely depending upon the type of cancer from which they arose. Treatment varies according to where it started and the extent it has spread, among other factors, including the patient’s general health, age and response to previous treatment.

Treatment for primary brain tumors and metastatic tumors can include radiation, chemotherapy and surgery. For both primary and metastatic tumors, serial follow up treatment will usually be necessary. This includes exams and MRI scans to detect recurrence or any long-term effects of the tumor or treatment.

Whenever possible, surgical resection (surgery brain tumor) will be the choice of treatment, depending on the size and location of the tumor. An incision is made on the scalp, and bone covering the area of tumor is removed (known as craniotomy), and the tumor is removed. The majority of patients then stay in hospital for 2 to 7 days post surgery, depending on different on location, tumor malignancy and general health status.

After surgery, fractionated radiation therapy may be used on tumors such as astrocytoma, glioblastoma, oligodendroglioma, ependymoma, medulloblastoma, as they are infiltrative, meaning even with all of the visible tumor removed, microscopic disease will remain.

For benign tumors such as meningioma and schwannoma, or metastatic tumors located in difficult areas to the surgical approach, radiosurgery is another treatment option. This technique uses a singular very high dose of radiation which is delivered to the tumor but does not affect surrounding brain structures. It is only appropriate for tumors that are separate from the brain and less than 3cm in diameter, and easily delineated on MRI or CT imaging.

Treatment of Metastatic Tumors: surgery of brain metastasis, radiosurgery, chemotherapy and fractionated radiation, are all options to treat metastatic tumors and they are often used in combination. Minimally invasive neurosurgery is a global philosophy which aims to achieve more effective and efficient surgery, with the objective of neurological and psychological function preservation, therefore lowering the morbidity rate. This philosophy can be applied to all neurosurgical procedures, be it brain or spine surgery.

Two fundamental aspects of the philosophy are:

  • To perform better operations;
  • To achieve better results, in a safer way and with less blood loss for patients.

Two types of minimally invasive neurosurgery are Keyhole Neurosurgery and Neuroendoscopy.

Keyhole Neurosurgery

For certain brain tumors deep under the surface of the brain, a narrow opening with a diameter of approximately half an inch can be used to reach it. The narrow opening, accessed using a minimally invasive surgical technique, minimizes damage to normal brain tissue overlying the tumor or lesion. The Keyhole Neurosurgical philosophy includes not only limited craniotomy size but also minimal brain retraction.

In most of the cases, precise stereotactic neuronavigation can allow extremely small skin incisions and bony openings (craniotomy) for the removal of brain tumors.

Some of the tumors that possibly can be approached using minimally invasive “key hole” craniotomies include pituitary tumors, craniopharyngiomas and sphenoid wing meningiomas.

Neuroendoscopy

This technique involves using endoscopes (specialized narrow, flexible, lighted tubes, only 4mm in diameter, with high resolution video cameras) to look at various parts of the brain, skull base or spinal cord through small openings. Endoscopes are effectively small microscopes, which magnify critical anatomical structures so that the various diseased areas requiring repair, removal, or replacement are easily viewed by the surgeon, allowing the surgery to be performed. The endoscope brings the viewing lens close to the working area, providing a high resolution, magnified view of the operating field; the tumor and surrounding important structures.

Technical advances in endoscope design have been substantial in recent years, including improved resolution, reduced size and brighter illumination of the endoscope.

Conventional surgery is much more intrusive than when using endoscopes, hence endoscopic neurosurgery being referred to as minimally invasive neurosurgery. Endoscopic neurosurgery in general does not involve large incisions on the scalp or skull, or extensive dissection through brain tissue, in fact, many brain tumor operations that previously required a large scalp incision and large bony opening (craniotomy) can be performed as minimally invasive surgery, using smaller incisions (usually less than an inch) and craniotomy openings. Special microsurgical instruments are used alongside the endoscope for precise dissection and tumor removal.

Intraoperative Magnetic Resonance Imaging (MRI) in Neurosurgery

What is it? Intraoperative Magnetic Resonance Imaging (MRI) is the use of an MRI scanner in the operating room during surgery, allowing the surgeon to take images of the patient. It is frequently used in Neurosurgery, especially for the removal of brain tumors.

How does it work and what are the benefits? Magnetic resonance imaging (MRI) uses the body’s natural magnetic properties to produce detailed images from any part of the body. It is available in a range of strengths. Low-field units, less than 1 Telsa (T), have the benefits of being small and portable with a simpler operating theater preparation however the image resolution is relatively poor. Higher field strengths are currently available in 1.5 and 3T options, and these allow better spatial and contrast resolution which means surgeons can assess the findings on an image more accurately.

When using Intraoperative Magnetic Resonance Imaging (MRI) the operating room can be configured in two ways. Both ways require the MRI magnet to be stored in an adjacent room, and one configuration is where the patient has to be moved to the magnet to obtain an image.

The second configuration is where the image is obtained by moving the MRI magnet to the patient via ceiling rails. This approach means that the patient can remain in the operating room during the surgery which allows a seamless workflow and increased safety in terms of monitoring, airway control and head fixation.

By providing MRI during neurosurgery the risk of damaging critical parts of the brain is reduced which therefore improves the rate of procedural success.

This state of the art technology, when used in conjunction with high-end microscopic surgery equipped with tumor fluorescence capabilities, enables certain brain tumors to be resectioned more precisely, therefore preserving maximum neurological functionality. It means that surgeons can distinguish between tumor tissue and normal tissue, minimize disturbance of healthy tissue or critical areas of the brain, evaluate and confirm if the operation has been a success, detect if there is residual tumor and make adjustments during a procedure, including additional resection before surgery is completed, without moving the patient (in the case of the rail-mounted configuration).

By using MRI technology there is a greater likelihood that a surgeon will remove the entire tumor than when surgery is performed in a conventional operating room where MRI is not used. Published clinical evidence shows the higher percentage of tumor removed the better the outcome.
